Book Description
History collection is an essential step before a diagnosis of the health problem/disease of the patient. It needs practice by all the student nurses, especially during their rst year clinical posting area. Sometimes they may confuse or anxious what kind of history to be obtaining and the sequence of history collection of the patient. The authors assume special attention needs from this area and try to help nursing students by preparing the basic history collection format. The format consists of collecting the patient's demographic data, the performance of head to toe physical examination, and components of basic guideline measurement sheets as vital signs recording, intake and output chart recording, diabetic chart recordings, and including nurses recording reporting chart. I hope this will help the young buds of nursing students who are entering the nobel profession.
