The primary goal of endodontic therapy is to create an environment within the root canal system which allows healing and continued maintainance of the peri-radicular tissues. Endodontic Mishaps can occur at any stage of the root canal treatment right from placement of Rubber dam to the Root canal lling and it is old saying that "prevention is better than cure".Biggest challenge in treating mishap is its detection which is a rst step in the management. After detection, correction may be accomplished depending on the type and extent of the procedural accident. The next step is to re-evaluate the prognosis of the involved tooth. This is very essential as it may affect the entire treatment plan and may also involve dentolegal consequences. If a mistake will affect the outcome of the case, the patient must be informed and the mistake must be noted in the record along with a statement that the patient was informed of the mistake and this should be followed as cardinal rule.
